The Care Co-ordination Service

Please find attached details of the Care Co-ordination Service (CCS) of Central London Healthcare, who provide the services being surveyed by Westminster Health Watch - see below

  • Are you, or someone you care for living with a health condition that is likely to last longer than 6 months?
  • Have you received extra support. either from your GP or another health professional from your surgery to manage your health condition?
  • Would you like to take part in a focus group to share your experiences of living with or caring for someone with a health condition that is likely to last longer than 6 months?

Primary care coordination services have changed in Westminster. Healthwatch and the Older Adults Group at the Advocacy Project want to hear your experiences of receiving support, so that we can check that the changes are improving people's lives.  See link to survey

Wellbeing Service -                                                                                          

This is a programme which offers patients a range of options to 
discuss and learn more about emotional health and wellbeing. They provide 
access to information sessions and workshops, send Invitations to local wellbeing events, can offer self help leaflets, emotional wellbeing checks and run
