You can complete the survey online (online services below) or a paper copy can be picked up at the surgery. The closing date for completing the survey online is 22nd Feb 2012 and the results will be discussed with the PPG and published to the website soon after.

You can now send us your travel health status using the online questionnaire on the website.

You can also let us know if you have changed your address, telephone number, order your prescriptions and book or cancel appointments online as well.

Whooping cough cases increase

Cases of whooping cough (pertussis) reported to the Health Protection Agency (HPA) in England and Wales have more than doubled from 421 cases in 2010 to 1040 in 2011.
